How should I make my first appointment?
Appointments can be made by calling our office during office hours. We are open Monday through Thursday from 9 AM to 6 PM and Friday from 9 AM to 5 PM.
New patient should arrive 15 minutes prior to their scheduled appointment in order to complete the required paperwork prior to your visit with the doctor. To be best prepared for your initial visit, please remember to bring an official personal identification card, your insurance card and your new patient forms. Payments can be made in the form of cash, MasterCard or Visa.
Which insurance companies participate with Lakeview Medical Associates?
We participate with most major insurance companies including, but not limited to, Aetna, Cigna, Horizon, AmeriHealth and Medicare.
Is there a fee for cancelling a scheduled appointment?
We reserve the right to charge for appointments cancelled or broken without 24 hours advanced notice. As a courtesy we do confirm appointments over the phone the day before your appointment is scheduled.
